Insurance Billing Policy
As a courtesy, after payment of your copay, we will bill all major insurance carriers except Kaiser. To assist with this billing courtesy, you will be asked to confirm your insurance information on record, and for any updates to your personal contact information each time you visit our office. NOTE: Regretfully, we will NOT bill third parties such as workman's compensation, auto insurance, or others who may be libel for your medical expenses.
The patient accepts primary responsibility for determining if “Peter Prutch” or “Family Centered Medicine” is in-network for their chosen plan. If a claim processes as out-of-network, the patient agrees to current self pay rates.
Please understand that Family Centered Medicine is not responsible for and will not waive payment for your health insurance plan’s required co-pay, deductible and/or co-insurance:
Please direct any questions you may have regarding your co-pay, deductible and/or co-insurance for which you are responsible to your health insurance administrator. The phone number and/or website to contact them is usually on the back of your health insurance card.

The patient is solely responsible for contacting their insurance carrier to confirm if our office is in-network.
